Physical & Athletic Forms
These are the approved forms for the current school year. Please read each section carefully to determine which files you will need to download and complete for your child/children in the current school year.

Your child's athletic coach can answer any questions you may have about the forms required for sports participation. Please see the Athletics section in either the Middle School or High School area of the website.
Grade-Specific Physical & Screening Forms
Dental Screening
State law mandates that children entering both Kindergarten and 9th grade complete a dental screening. This form should be filled out by your dental office.
Pre-School Physical
Use this form for children entering preschool. Physicals done within 12 months of school starting are acceptable.
Kindergarten Physical
Please use this form for children entering kindergarten. Physicals done within 12 months of school starting are acceptable.
Grades 3, 6, 9 School Physical
A physical is requested for children entering 3rd, 6th, and 9th grades.
