Welcome to the Nurse's Office

Schools, health agencies, parents, and communities share a common goal of supporting the link between healthy eating, physical activity, and improved academic achievement of children and adolescents. Evidence shows that the health of students is linked to their academic achievement, so by working together, we can ensure that young people are healthy and ready to learn.

  1. For grades Pre-K to 8th grade, please email the Elementary/Middle School nurse with your child's health or medication updates or with any questions or concerns.

ALLERGY & ASTHMA

 
Asthma Inhaler Consent

Please fill out the Asthma Inhaler form (2 pages) with information on your child's inhaler. The form requires a signature from both the parent/guardian, as well as the physician. This form will enable your child to carry their inhaler with them at school.

EpiPen Self Carry

This form must be completed and returned to the school nurse to allow a student to have and administer this medication at school.

Food Allergy Emergency Plan

This form must be completed and returned to the school nurse for students who have food allergies requiring an EpiPen.

 

 

PRESCRIPTION & OVER-THE-COUNTER MEDICATIONS​

 
Prescription Medication

This form enables the school to administer medications to your child.  Both a parent/guardian and prescriber need to sign and complete this form. Please bring this form with a labeled medication container to school.

Over-the-Counter Medication

Acetaminophen (Tylenol), Ibuprofen (Advil), Diphenhydramine (Benadryl), and Calcium Carbonate (Tums) may be taken while at school with parental permission and will be provided by the school nurse. Medications may be added or deleted from this authorization form at any time during the school year by contacting the School Nurse. A new form must be signed and submitted each year.
